early after open heart surgery(cabg),in the icu,suddenly:hypotension,sinus tachycardia and suspicion of anemia.urgent TEE was done and shows:large localized pericardial effusion(hematoma,confirmed by surgery),compressing right atrium and right ventricle(tamponade) with multiple fibrin strands inside.
