In this TEE clip prior to an electrophysiology procedure for atrial flutter, the left atrial appendage (the elongated dark structure running up an down) is examined for evidence of thrombus. None is seen here, but if there was it would be a mildly heterogeneous substance seen in the dark area with echodensity somewhere between the black of blood and the gray of the myocardium. Pulse wave doppler can also be used to establish that there is flow into and out of the LAA.
