This apical TTE clip demonstrates the appearance of a patient with a severe dilated LV cardiomyopathy that has resulted in distortion of the mitral valve, severe prolapse, and regurgitant flow.
In this apical TTE clip, the posterior mitral leaflet can be seen prolapsing into the left atrium and completely failing to coapt with the anterior leaflet. Furthermore, there appears to be a ruptured
chordae and part of the leaflet is flail. In another clip we will see that this results in severe mitral regurgitation.
In this zoom view of the mitral valve, we see the valve with and without color flow doppler. In this case, the regurgitant flow occupies the entire left atrium, which is not enlarged, suggesting a
relatively new onset of regurgitation. The prolapse of the posterior leaflet results in a jet directed towards the interatrial septum where the flow circulates in the atrium. This amount of MR also
resulted in reversal of pulmonary venous flow.
This apical TTE clip without doppler shows the enlargement of the left ventricle (due to AI) and the left atrium (due to MR) in a patient with longstanding valvular disease. Notice the bulging of the
atrial septum to the right consistent with significant increase in left atrial pressure. Also note the profound prolapse of the posterior leaflet of the mitral valve.
