A ligated Left Atrial Appendage
Prior to cardioversion, this patient with prior CABG supposedly had the LAA ligated. The indentation in the atrium is the remnant of the LAA. Notice the faint spontaneous echo \smoke\.

Aortic dissection in parasternal long axis TTE
Frequently, the descending aorta can be seen just behind the left atrium in the parasternal long axis view on TTE. In this case, a subtle finding is a serious one that must be detected. The linear
echodensity across the aorta is an acute dissection of the aorta.

Aortic insufficiency in a patient with dissection
In this TTE zoom view of the aortic/mitral valves in the parasternal long axis with color doppler, you can see dramatic aortic insufficiency. Even more worriseome is the aorta, seen just behind the
left atrium. Note that the circular structure has a linear density across it, which is a dissection flap and the cause of this patient\s acute aortic insufficiency.

Echo "smoke" on TEE
In this TEE clip prior to an electrophysiology procedure for atrial flutter, spontaneous echo contrast or "smoke" can be seen wisping through the left atrium due to blood pool stagnation
from the atrial dysrhythmia.

Huge Inferior Pseudoaneurysm 1 of 5
This is the first in a series of clips from a patient with a very large inferior pseudoaneurysm that communicates with the RV and with a large pulsatile mass behind the LA compressing the atrium. The
findings were also verified on Chest CT and invasive angiography. It is not known when this occurred in the patient, but he did undergo bypass surgery several years ago and recently suffered another
MI.

This is the second in a series of clips from a patient with a very large inferior pseudoaneurysm that communicates with the RV and with a large pulsatile mass behind the LA compressing the atrium. The
findings were also verified on Chest CT and invasive angiography. It is not known when this occurred in the patient, but he did undergo bypass surgery several years ago and recently suffered another
MI. This is 2 chamber apical without doppler.

Huge Inferior Pseudoaneurysm 3 of 5
This is the third in a series of clips from a patient with a very large inferior pseudoaneurysm that communicates with the RV and with a large pulsatile mass behind the LA compressing the atrium. The
findings were also verified on Chest CT and invasive angiography. It is not known when this occurred in the patient, but he did undergo bypass surgery several years ago and recently suffered another
MI. This is a 2 chamber apical clip with color flow Doppler.

Huge Inferior Pseudoaneurysm 4 of 5
This is the fourth in a series of clips from a patient with a very large inferior pseudoaneurysm that communicates with the RV and with a large pulsatile mass behind the LA compressing the atrium. The
findings were also verified on Chest CT and invasive angiography. It is not known when this occurred in the patient, but he did undergo bypass surgery several years ago and recently suffered another
MI. This is a parasternal long axis clip without Doppler that demonstrates the pulsatile mass of blood collecting behind the left atrium.

Huge Inferior Pseudoaneurysm 5 of 5
This is the last in a series of clips from a patient with a very large inferior pseudoaneurysm that communicates with the RV and with a large pulsatile mass behind the LA compressing the atrium. The
findings were also verified on Chest CT and invasive angiography. It is not known when this occurred in the patient, but he did undergo bypass surgery several years ago and recently suffered another
MI. This clip shows a small Doppler jet going into the right ventricle.

Normal Bubble Study
When agitated saline is injected into the venous system, it opacifies the right atrium and ventricle. None should make it through to the left side of the heart. If bubbles appear on the left
immediately, it is indicative of an intracardiac shunt, while bubble that appear within a few heartbeats are consistent with an intrapulmonary shunt. Neither are seen in this TTE loop.

RA collapse, is this tamponade?
In this apical TTE clip, the right atrium is collapsing. It is a little hard to say whether this is occurring as a result of atrial systole or during ventricular systole consistent with tamponade.
